Monitoring and Adjusting Plans: They provide guidance on how to make adjustments to supports as needs change over time, ensuring the participant’s plan remains relevant and effective.

Begin by submitting an Access Request form. This can be done online, by phone, or through mail. The form will ask for details about your disability, and you may need to provide supporting documentation from medical professionals.

After submitting your application, the NDIS will review your eligibility. If accepted, you’ll be contacted to set up a planning meeting with an NDIS planner or Local Area Coordinator (LAC) to discuss your needs, goals, and desired outcomes.

In this meeting, you’ll work with an NDIS planner or LAC to discuss your daily support needs, current services, and future goals. Together, you’ll identify supports that would help you, which will be included in your personalized NDIS Plan.

An NDIS Plan is a personalized document that outlines the types and amounts of support funding you’ll receive based on your goals and needs. This plan is funded by the NDIS to help you access the services and supports that best suit you.

Once your plan is approved, you can start using the allocated funds to access services and supports. A Support Coordinator or Local Area Coordinator can help you connect with providers and manage your plan.

Yes, NDIS plans are reviewed regularly (usually annually) or whenever significant changes in your needs occur. During a plan review, you can discuss any changes needed and request adjustments to your plan.

To be eligible for NDIS funding, you must meet the following criteria:

1 Age: You need to be under 65 years of age when you apply.

2 Residency: You must be an Australian citizen, a permanent resident, or hold a Protected Special Category Visa.

3 Disability: You must have a permanent and significant disability that impacts your ability to perform everyday activities without assistance. This includes physical, intellectual, sensory, cognitive, and psychosocial disabilities.

4 Early Intervention: In some cases, individuals with a disability or developmental delay may qualify for early intervention support if it is likely to improve their outcomes or reduce the impact of the disability over time.

You can use the NDIS Eligibility Checklist on the NDIS website or contact the NDIS directly to discuss your specific circumstances and determine if you qualify.
